I'm thrilled that you are here and excited to help you see what's possible with your health. This application is for a 30 minute phone Strategy Session with me, Dr. Angela Carlson. It is the beginning of the enrollment process for my private membership practice.

During our 30 minute phone Strategy Session, I'll ask specific questions about your symptoms to see if my holistic, functional medicine approach is helpful for your case. If we are a good fit to work together, you'll be invited to join membership and book your initial appointment. If we are not a good fit, I'll do my best to recommend another practitioner that may be able to help you.

My holistic functional medicine practice is very unique and innovative. It's less of a "doctor's visit" and more of a "guided partnership" to sequence your steps to better health. Therefore, I only invite those action-takers that are ready to make the necessary nutrition and lifestyle adjustments to help themselves uncover better energy, deeper sleep and improved vitality. My membership programs offer you additional support with group sessions, online classes, and videos so that you have the support you need every step of the way.
